Our Philosophy
At DMOC, we believe that every person has the capacity to grow, recover, and live a life that has meaning and purpose. Our role is not to manage people — it is to walk alongside them, provide the right support at the right time, and gradually reduce dependency as confidence and capability grow.
Person-centred support means starting with the individual. Before any support begins, we take the time to understand who this person is — their history, their strengths, their goals, and their preferences. We co-produce every support plan, and we review it regularly to ensure it remains relevant and useful.
We work collaboratively with families, carers, statutory services, and community organisations — because good outcomes almost always require good partnerships.
Our 6-Step Support Pathway
Referral Received
We receive your referral online, by email, or by phone. We acknowledge all referrals within 1 business day.
Initial Assessment
A member of our team contacts the referrer and, where appropriate, meets with the individual to assess need, eligibility, and fit.
Care Plan Developed
We co-produce a personalised support plan with the individual, setting clear goals, identifying the right package, and agreeing the frequency and format of support.
Support Begins
Support commences with a named key worker. We work at a pace that suits the individual, building trust before introducing new challenges or goals.
Regular Reviews
We review all support plans at a minimum of every 12 weeks — or sooner if circumstances change. Reviews involve the individual, their key worker, and relevant professionals or family members.
Transition / Ongoing Support
As goals are met, support is stepped down in a planned and supported way. Where ongoing support remains necessary, we continue to provide it — adapting the plan as needs evolve.

Reviews & Ongoing Support
We review every support plan at a minimum of every 12 weeks. Reviews are structured conversations — not tick-box exercises. We look at what is working, what has changed, and what goals need to be updated or added.
Family members and carers are involved in reviews where the individual consents to this. We believe that the people who know and love someone are often the most valuable source of insight into their progress and needs.
Where circumstances change significantly — a housing move, a change in mental health, a new goal — we conduct an unscheduled review and update the plan accordingly. Support is never static at DMOC.
