Four structured programmes designed around the individual — not the system.

What DMOC Offers

DMOC Support Services provides community-based support for adults 18+ in Wolverhampton and the West Midlands. Our four support packages are designed to meet people where they are — whether transitioning from inpatient care, working toward independence, following a recovery pathway, or navigating the shift from care to adult life.

Every package is delivered on a person-centred basis. We work with the individual, their families, and professional networks to create a support plan that reflects their goals, builds on their strengths, and respects their autonomy.

Our Services in Detail

Step-Down Support +

Step-Down Support is designed for adults transitioning from inpatient mental health units, residential settings, or hospital care back into community living. This is often a high-risk period, and the right support at the right time can make the difference between sustained recovery and readmission.

Our Step-Down Support includes:

  • Regular structured check-ins and welfare visits
  • Support with establishing daily routines and structure
  • Assistance with medication management reminders
  • Liaison with community mental health teams and GPs
  • Support with housing, benefits, and community reintegration
  • Crisis prevention planning and early warning sign monitoring

Step-Down Support is typically provided on a short-to-medium term basis and is regularly reviewed as the individual builds confidence and independence.

Independent Living Support +

Independent Living Support helps adults who have the potential to live independently but need structured support to develop and maintain the skills and habits required. This may include individuals who have experienced prolonged periods of supported accommodation, those managing long-term mental health conditions, or adults who need help sustaining a tenancy.

Our Independent Living Support includes:

  • Support with budgeting, bills, and household management
  • Help accessing education, training, or volunteering opportunities
  • Building community connections and social networks
  • Tenancy sustainment and support with landlord communication
  • Life skills development (cooking, cleaning, time management)
  • Signposting to relevant local services and benefits

This package is flexible in intensity and can be stepped up or down based on assessed need.

Recovery Pathway Support +

Recovery Pathway Support is a structured, goal-focused package for adults who are working through a recovery journey — from mental health difficulties, and who need consistent, non-clinical support alongside their treatment or therapy.

Our Recovery Pathway Support includes:

  • Co-produced recovery plan with clear, achievable goals
  • Regular key worker sessions with progress reviews
  • Relapse prevention planning and early intervention
  • Support to engage with psychological therapies and community support groups
  • Employment and social participation goals
  • Family and carer involvement (where appropriate and consented)

We take a strengths-based approach, focusing on what the individual can do and wants to achieve, not simply on managing risk.

Young Adult Transition Support (18+) +

The transition from care to independent adult life is one of the most significant and challenging periods a young person can face. Young Adult Transition Support is specifically designed for those aged 18 and over who are leaving, or have recently left, the care system — and who need structured support to navigate adult services, build independence, and avoid crisis.

Our Young Adult Transition Support includes:

  • Named key worker who provides consistent, trusted support
  • Help navigating adult health, housing, and social care systems
  • Practical skills for adult life (budgeting, cooking, tenancy management)
  • Support to access education, training, apprenticeships, or employment
  • Social and community integration support
  • Emotional support and advocacy

We understand the particular vulnerabilities of this group and work closely with leaving care teams and housing providers to ensure a safe, supported transition.

Important: What DMOC Does Not Provide

DMOC Support Services provides community-based non-clinical support. We do not provide:

  • Personal care (washing, dressing, or other intimate care tasks)
  • Medical or nursing services
  • Crisis intervention or out-of-hours emergency support
  • Overnight or live-in support
