We work with adults 18+ in Wolverhampton and the West Midlands who need structured community support.

Adults with Mental Health Difficulties

Many of the adults we support are living with mental health conditions — including depression, anxiety, bipolar disorder, personality disorders, or psychosis — and require consistent, structured support in the community to maintain their wellbeing and avoid hospital admission or readmission.

Our support is not clinical or therapeutic — we do not provide counselling or psychological interventions. Instead, we provide the day-to-day human contact, practical assistance, and structured routine that helps people manage their lives alongside their condition.

We work closely with community mental health teams, GPs, and other statutory services to ensure our support is coordinated, appropriate, and genuinely helpful to the individual.

Adults with Mild Autism

We support adults who are autistic and who would benefit from structured community support to develop independence, build skills, and navigate a world not always designed with them in mind.

Our workers are attentive to individual communication preferences and support needs. We adapt our approach to each person, ensuring that support is predictable, consistent, and provided in a way that respects sensory sensitivities and routine.

Support may include help with community participation, accessing employment or education, managing day-to-day tasks, or building social connections — always in line with the individual's own goals and priorities.

Young Adults Leaving Care (18+)

For young adults moving from the care system into independent life, the transition can be abrupt and overwhelming. Many face this period without the family networks that their peers can rely on, while simultaneously navigating new responsibilities — housing, finances, employment, and adult services — often for the first time.

DMOC provides a consistent, trusted key worker relationship during this period, offering practical support alongside emotional stability. We work in close partnership with leaving care teams to ensure young people don't fall through the gap between children's and adult services.

Our support is flexible and responsive — we understand that this group's needs can fluctuate significantly, and we adapt accordingly.
