We accept referrals from professionals, individuals, and families. All referrals are reviewed within 1–2 business days.

Who Can Make a Referral?

Professionals

We accept referrals from any professional working with the individual, including:

  • Social workers and care managers
  • NHS staff (GPs, community mental health teams, hospital discharge teams)
  • Housing officers and support workers
  • Leaving care and personal advisors
  • Probation and justice teams

Individuals & Families

You don't need a professional referral to contact us. Individuals and family members can refer directly:

  • Self-referrals from adults 18+ in the West Midlands
  • Family or carer referrals (with the individual's knowledge)
  • Informal enquiries — we'll advise on whether we can help

Note: We may request a professional to be involved at the assessment stage.

Eligibility Checklist

Please review this before completing the referral form. If you're unsure, contact us and we'll advise honestly.

The individual is aged 18 or over
They live in Wolverhampton or the West Midlands
They have a mental health condition, mild autism, or are leaving care
The support needed is non-clinical and community-based
The individual is not in immediate crisis requiring emergency services
Personal care, nursing, or overnight support is not required
Out-of-hours crisis intervention is not the primary need

Service Notice

DMOC Support Service provides community support and independent living assistance only.

We do not provide personal care services such as bathing, dressing, or medical care.

Our services support adults aged 18 and over to maintain independence and stability within their communities.

Our team is available during office hours to discuss a referral, answer questions about eligibility, or provide advice on alternative services if DMOC is not the right fit.

Please note that DMOC does not provide personal care, medical or nursing services, crisis intervention, or overnight support. If the individual requires any of these, please contact the relevant statutory service.
